Type all the possible answers into the 'Text' box, hitting 'enter' after each one.  Answers will appear in the 'Responses' box on the left.  Then click on the fifth answer 'Other' and click on the 'Write in' box  in the centre of the screen to allow respondents to enter a verbatim answer.
